Individual Health Risk Assessment
 
These are the steps for individual:

Step 1. Attend a blood draw and attain your biometrics

Step 2. Within two weeks, your blood draw results will be mailed to your home. On the back of the results page there are directions on how to access the Mayo Clinic, BEWELLEMBODYHEALTH web portal.

Step 3. Go online to www.bewellembodyhealth.com and click on the button that says, “Take your HRA” now. This process usually takes 15-20 minutes.

Step 4. After you have finished your online Health Risk Assessment, HRA, you are able to print your results. You now have information needed to start becoming a healthier you.

Step 5. Now that you know your results there are many different ways to move forward.

  • Join a health club- WAC information here
  • Take your results to your next doctors appointment
  • Use the online tools at www.bewellembodyhealth.com to work towards a healthier you.
  • Access Mayo- Clinic’s ask a nurse hotline, call WHF for information on cost a availability
