NCSD Employees/Retirees to the Group Insurance
office CIGNA Insurance for the following tests: |

Chemistry Profile
Hemogram
PSA for Men
Hemoglobin A1C |
Ferritin
C-Reactive Protein
Vitamin D
ColoKit |

IMPORTANT:
Present your
CIGNA id. Card!
Each
employee, retiree, or dependent is eligible for
blood screenings (listed above or posted at each
site), asdescribed below:.
- DO NOT PAY IF:
- This is your 1st blood draw
of the year present your CIGNA id card.
- You are on the Health Plan.
- Your spouse and/or children
are on the Health Plan and present their
CIGNA id. card.
- You require a subsequent
blood draw, have a doctor's note, and
present your CIGNA id card.
- PAY AT THE TIME OF
YOUR APPOINTMENT IF:
- You and/or your family member
do not have your CIGNA
id. card.
- You and/or your family member
are not on the Health
Plan.
- You receive a blood screening that
is not on the
eligible list at each site.
